### Summary
The Northeast Ohio VA Healthcare System is recruiting for a Social Worker. The Social Worker will function in the Home Based Primary Care Team (HBPC), Community Outpatient Service, based out of the Wade Park Medical Center in Cleveland.
**Work Schedule:** Monday through Friday, 8:00 a.m. - 4:30 p.m.

### Responsibilities
The Social Worker duties include:
* Conducting independent home visits in Cleveland and eastern suburbs to veterans with complex medical and psychosocial conditions.
* Developing assessments in collaboration with interdisciplinary treatment team, veterans, and family members.
* Completing assessments to highlight the veteran's strengths, limitations, and internal/external support and service needs by making home visits primarily in inner city neighborhoods.
* Developing crisis management plans to maintain patient in the home, for admission to acute, short term and/or long term placements.
* Providing veterans and their caregivers with counseling to deal with the psychosocial impact of coping with chronic/disabling illness(es), onset of a catastrophic illness and/or dying, as needed during the course of the veteran's enrollment in the HBPC Program.
* Reassessing veterans needs for non-institutional and institutional services/programs and entitlements.
* Educating the veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the veterans preference.
* Developing resource files of VA and community social service programs to refer the veteran to needed services.
* Participating in weekly care planning meetings.
* Establishing and maintaining positive working relationships with employees, volunteers, consumers and stakeholders within the VA and outside community agencies.

### Qualifications
**Basic Requirements**:
* United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
* Must have a Master's Degree in social work from a school of social work accredited by the Council on Social Work Education.
* Must be licensed or certified at the Master's level to independently practice social work in a state.
* Must possess a valid driver's license.
**Grade Determinations**:
*GS-11 Level:* Applicant must possess one year of post-MSW degree professional social work experience in the field of health care and be licensed or certified in a state at the independent practice level. This experience must have been in a clinical setting and must have demonstrated the potential to perform advanced assignments independently. (A clinical setting is a medical center, clinic - medical or psychiatric, residential treatment center, or any other type of facility where social work participates in a collaborative treatment and is identified with the medical profession.) Experience must have been obtained after earning a Master's Degree in Social work.
*Substituting education for experience at the GS-11 level:* To meet the required specialized experience for the GS-11 level, applicant may substitute a doctoral degree in social work from a school of social work. You must submit a transcript to receive credit for this education. A doctoral degree in social work may not be substituted for the master's degree in social work.

The full performance level of this vacancy is GS-11. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-09 to GS-11.
**Physical Requirements:**Applicant must possess the ability to climb stairs; lift, carry, pull, and push up to 25 pounds; frequent walking, standing, kneeling; repeated bending, squatting; and driving a car for periods up to 2 hours.
